Hi, I'm Dr. Derrick Johnson, and I wanted to talk to you a little bit today about TMJ.
TMJ stands for temporomandibular joint, and so that is this part of your anatomy.
It's basically your jaw joint hinge.
Sometimes you'll hear the term TMD, which is temporomandibular dysfunction or disorder.
But anyway, that's the topic today, and it's actually a pretty big topic, and the reason
I say that is this is the most used joint in your body, and if you look at, basically,
what does your head do, it only one mechanical function.
It opens and closes and moves the jaw.
So, all of your ... the major muscles of your head actually tie into movement of this joint.
And I want to show you, this is a piece of software.
It's really helpful.
It's called BiteFX.
But, as far as illustrating how the jaw works, what we have here ... Here's the actual joint
itself, and here's all the muscles that come together to move the jaw.
And what we want in health, we want this system to be in harmony and coordinated, and what
that means is when this joint fits with the disc, there's a disc in place, when the joint
fits, all the teeth fit, and your nervous system and the muscles, they like that when
things fit.
I'll often use the analogy of a door.
A door's another hinged system.
So, here's a door that, it fits very well, so it's very effortless to open and close
this door.
Hardly any effort, but you could imagine if that door was off the hinge, and imagine if
I had to open it every single time, it would take a lot of energy and a lot of muscle activity.
So basically, when this jaw joint is off the hinge, and it's not lining up with the teeth,
your body does this self-protective response, just like opening the door off the hinge.
The muscles are all tense all the time trying to coordinate the jaw joint.
And the problem with that is it can cause a lot of damage.
It can cause sore muscles, it can cause headaches, it can cause actual damage and deterioration
of the joint, joint noises.
It can cause ear pain.
It very, very typically causes tooth wear, breaking of teeth, tooth loss, root canals.
There's so many things that can be traced back this system being out of whack or out
of order.
And so, that's the basic idea of how we treat this.
So, if you're suffering from headaches, clenching your teeth, a history of broken teeth, multiple
root canals, worn teeth.
Sometimes there is like a ringing in the ear or a fullness in the ear can be one of the
symptoms as well.
A lot of times there's neck pain associated, like neck and TMJ pain very much go hand in
hand.
A lot of times a history of a car accident or whiplash injuries almost always affect
the TMJ to some extent.
So, those are some ways that it shows up, but one of the first things that we want to
do is evaluate that system, and here we can take a look at it again.
We want to know when the joints are in place, are the teeth in alignment?
And another tool we use to figure that out is we have something called an articulator,
so we could actually build a replica of how your jaw works, and we can see okay, what
are your joints and teeth doing?
Are they in harmony?
Are they out of harmony?
We can start to see wear patterns on the teeth and kind of do the detective work to figure
out what's the root cause of the problem?
So essentially, what the fix is is there's techniques where we can get the system back
in harmony, and the sooner that we do that when there's signs of a problem, the sooner
that we can do it, the more effective, easier, less expensive the solutions are.
So anyway, that's a little bit about TMJ.

of motion before was a lot more painful for her
with the exercise we've done here it's helped her increase that range
of motion and that data has been saved

motion was initially and where she's at now

Medication Assisted Treatment for Opioid Dependence - Duration: 2:59.
We have a couple of common medications that are used to treat opioid dependence, both
methadone and buprenorphine, it's often a combination product, buprenorphine/naloxone.
So essentially there's these couple of products that are available and there's been lots and
lots of research with these products over many many years showing that they are the
most effective way to treat opioid dependence.
These medications either methadone given as a liquid or buprenorphine/naloxone often taken
sub-lingually, so either a tablet or just a film that goes under the tongue.
These medications are long acting medications, so you only need to take them once a day with
methadone or even once every couple of days with buprenorphine.
And when you take those medications, it reduces those feelings, or usually gets rid of those
feelings of craving, and thinking about other opioids, and also those physical symptoms
of opioid withdrawal, so a person feels well in themselves.
And they don't have those constant thoughts about opioids.
Particularly around the time that people were having a lot of problems with over-the-counter
codeine, there were a number of people that I spoke with through my research and they
would spend, you know, many hours of the day, every day, going to multiple pharmacies to
acquire codeine, and they'd never heard of this treatment, buprenorphine, and so when
they finally started on it they had you know, a bit of hesitation and they had their first
couple of doses and then, there was this realisation that, "I had this one dose of medication,
once a day in a pharmacy, and I feel fine for the rest of the day."
And I guess all of the problems and the worry about running out of medication, the worry
about feeling sick, the overwhelming feeling that people expressed to me was just one of
relief.
That there is a treatment and that it works.
And you know, now they feel great and they can't believe it took so long to try these
medications.
But I can understand that there's a bit of apprehension about this new treatment that
maybe people have never heard of, so I think part of the importance is getting the message
out there that there are treatments that work.
What people tend to say that they feel when they take these medications is that they feel
normal.
So these medications, while they are still in the family of opioid medicines, because
they're such long-acting and sort of slow-release opioids, they don't tend to have people feeling
euphoria or the term 'high', so that's not something that you feel on these medications,
so people just feel normal, they're not experiencing opioid withdrawal anymore, and they're able
to get on with their lives.

Treatment options benzodiazepines - Duration: 2:01.
Benzodiazepines, particularly if people have been taking them for months or years, can
be a little bit tricky to stop.
And so, again, really important to talk to a health professional.
Some of the best evidence we have around the approaches to stopping benzodiazepines suggest
that reducing the dose very slowly, over a long period of time.
So we're not talking about a week, sometimes we're talking about a month to years.
And if you've been taking a medication for many years then that's understandable, it
might take a while for the body to get used to not taking those medications.
The other thing to often consider is, when those medications are being taken, so for
example, if you've been taking your medication for anxiety, or because you have difficulty
sleeping, sometimes the original cause of those things is still in your life, and until
you've got some strategies to manage that, to manage what to do when I feel stressed
if I'm not going to take a benzodiazepine, it can be difficult to reduce the medication
and that's why often the combination of both reducing the medication, but also getting
some psychological treatment, developing very specific strategies for, 'what do I do when
I'm feeling like this?
What's some other things that I can do other than take this medication?'
In general, we really discourage against just stopping cold turkey with benzos, and it doesn't
matter if it's a high dose or a low dose, because either way it will increase the risk
of difficult withdrawal symptoms.
But of particular concern is people who are on high doses and who have been on high doses
for longer periods of time, there is an increased risk for seizures if they stop cold turkey.
So that's something we really need people to be aware of, and to manage that, it might be
necessary that they go into an in-patient facility to manage the high dose to begin
with.
Even at low doses, stopping cold turkey is really going to increase the risk of withdrawal
symptoms full-stop.

Treatment options for opioid dependence - Duration: 1:57.
If you have concerns about the medications you're on and wondering about stopping, there
are a few suggestions I would make.
It's good to get some background information about that, and then having done that, it's
a matter of having a conversation with your prescriber and asking them about what
they think would be the most appropriate way, which can be individualised to you, to stop
that medication.
Our team usually talk about the main treatment being opioid deprescribing if the main underlying
problem is pain, that's because of the scientific evidence pointing to the fact that these drugs
are not effective or safe long-term treatments for pain.
The treatment of opioid dependence is more often, at least for a time, is maintenance therapy.
So that's a conversation really with your prescriber about what is the main problem that is being
dealt with here.
There's a range of different strategies, we have medication-based strategies, and that
can be either in the short-term, so people can have medications for maybe a week, or
ten days or a couple of weeks to reduce off,
or often they might consider taking medications for a longer period
of time, and that's going to depend on the person and what's going to work for them.
Also, at the same time, considering things like psychological treatments, and that might
be in the context of learning other strategies to manage your pain, or strategies to manage
when you feel opioid cravings, or when you're feeling distressed.
So having those things at the same time combined with the medications can often be the most effective
approach.
There are lots of ways that we can help, but you need to go and ask for that help.
Go and talk to your health professional and get the process started.

So my mate and I had restored a
1957 I think it was Mark One Zephyr
It was a convertible and we loved racing down the dead-end street as if we were going to crash into them
while they were playing cricket, just short of them and stopped just short of their wicket
We didn't do it too often because we were scared of their parents and
potentially the police turning up, but it appealed to our teenage sense of humor
We were all teenagers, right?
Now I can relate to the male balding pattern because while at the age of 17 I could sit on my hair
I was already showing
signs of a receding hairline
Hogan, or Martin Crowe as most people knew him, was following on from his brother, who like me started receding at quite a young age and
he wanted to stop this progression, and keep his youthful good looks
He therefore took on a hair transplant
and was it success and being one of the greatest cricketers of all time
he became an influencer for that treatment and
he was often seen on TV in a different role from scoring runs all over the world.
Sadly a few years ago, he lost his hair again. This time due to cancer.
I raised this for two reasons
Firstly with the greatest of respect,
because he went through some terrible times before he sadly passed away.

When I had hair it used to grow backwards. That way.
I guess because I brushed it that way and because the weight of my long hair in my early years
probably made the follicles lean in that direction
So this is how I hold the blade and how to do it
So the first thing is when you shave, quite often you might hold your blade down there. That's a handle, right?
That's what it's for. But the trick that he showed me was you hold it like that, so you've actually
got your fingers right against the back of the blade and you go like so
and in this case as I said
I'm following the grain at my head. That allows you really good control of the blade and you're not likely to cut yourself
So that's how I hold the blade and how I do it
I'm not hiding my head because of
the baldness. I'm quite happy being bald and everyone knows me that way it's simply it's still winter here in New Zealand and
my cap's keeping me warm and tonight
I'm hoping the Vodafone Warriors are going to go for another win, the New Zealand Warriors. And so I'm supporting them.
But it's actually just slightly too warm to wear my Warriors jacket, which I'll be wearing later. That's a totally different story. So

Recognising and seeking treatment for benzodiazepine dependence - Duration: 1:43.
If you're worried about your medication and that you might have developed dependence,
there's a couple of things.
Firstly, when you take medications every day for a long period of time, your body does
get used to them, and if you stop them suddenly, you often will experience withdrawal symptoms.
For that reason we don't recommend to just stop your medications suddenly without talking
to a health care professional.
It can either make you feel just pretty unwell, or sometimes there can be risks with sudden
cessation of medications like benzodiazepines.
So the first thing to do if you're concerned, is talk to a health care professional and
develop a plan to gradually reduce those medications.
As you can imagine, if the first symptoms that people experience when they start to
reduce their dose are actually intensifying of their initial problem, that's a reason
why many people actually believe that they still need the medication, and will continue
on taking it or potentially even increase their dose to try and alleviate those symptoms.
So it's really essential that before people attempt to start reducing, that they have
adequate support in place, and they know what to expect.
Because they are going to have to tolerate some periods of discomfort, but it really
shouldn't be too uncomfortable if it is a very gradual reduction.
Where the reduction is too rapid, or where people stop cold turkey, that's where we start
to see higher risk and the really problematic symptoms start to emerge.
So if people are on high doses, and stop cold turkey, they could actually have seizures.

and therefore reducing pain and activity.
So if say that getting a lot of pain from clenching their jaws
at night having facial muscle pains
or if they're getting temporal headaches
you can actually put Botox into these areas
and it will almost deaden the muscle down
and actually calm everything down.
So it's a very quick, short term and effective
immediate treatment.
But if those muscles are extremely tense
they'll need a really high dose because these are big muscles,
and they're big movements.
They're not like wrinkle relaxers you need a big dose.
So it makes it rather expensive to do
on a regular basis and ongoing.
And also, you know I like to treat things
very holistically and very naturally
and long term use of it is not really been tested.
So I am actually more on the side
of trying to treat the cause and stopping the problem
from coming up rather than symptomatic treatment, which
Botox is ultimately, it needs to be repeated between three
and six monthly and it becomes rather expensive.
But is effective for real quick short term pain relief.
