Have you ever been to the doctors with low back pain and been diagnosed as having a condition

Rheumatism is a little bit like lumbago, it's one of those old-fashioned terms to describe
low back pain of some shape or form without being really any more specific about what's
causing it or indeed how to treat it effectively.
So years ago often you go to the doctor with a flare-up and they would describe, or they
would diagnose, you as having rheumatism and they'd send you off with perhaps a course
of painkillers or if there were a good GP a course of anti-inflammatories and you'd
take those and nine times out of ten with a little bit of bed rest or perhaps a little
bit of exercise the symptoms would resolve and then you carry on as normal.
The modern day the modern day alternative to rheumatism is osteoarthritis, or degenerative
joint disease of the lower lumbars and what happens with osteoarthritis is that over a
period of time we experience a little bit of wear and tear in the joints and a little
bit of shrinking of the discs which cushion the vertebral bodies and as we get older the
discs thin and the joint surfaces start to approximate, they get closer together, and
we just get a little bit of wear and tear.
Now it's a normal part of the aging process and it's not necessarily a problem and often
providing that the joints don't become irritated through excessive movement or excessive exercise
then we don't feel this condition often we can live with a little bit of wear and tear
in our bodies and we don't feel it at all.
So it's not necessarily a bad thing, it's not necessarily something to worry about.
Pain associated with arthritic degeneration comes from inflammation.
So whenever something rubs together and becomes irritated we get inflammatory fluid and inflammatory
fluid is what causes the pain its not the wear and tear in the bones themselves.
So in terms of managing this condition if you can prevent the bones or the wear and
tear becoming irritation, there'll be no inflammation and no inflammation
means no pain.
For years this was often one of the problems associated with MRI scans you'd send the patient
for an MRI scan, the scan would show all kinds of degenerative wear and tear, all kinds of
disk problems, all kinds of bony anomalies and then that will become the justification
for the surgeon going in and removing all kinds of bits and pieces and it wouldn't make
any difference the symptoms at all.
So we know that we live with all kinds of structural defects, and they don't bother
us, they don't cause any pain they are actually called asymptomatic, which means it's a problem,
it doesn't cause pain.
Now we know that providing there's no inflammation, and there's no irritation pain will not be a result.
So providing that we manage our symptoms conservatively perhaps with a little bit of mobilization
therapy, a little bit of stretching and some strengthening, we can keep the area decompressed
we can keep the area loose and the wear and tear doesn't rub together.
It doesn't become irritated, and we don't get pain.
