Jason Grebely: Australia's success story - Universal access to hepatitis C treatment - Duration: 5:53.

My name is Jason Grebely.

I'm an Associate Professor at the Kirby Institute

at UNSW Sydney in Australia

and I'm also the President

of the International Network on Hepatitis and Substance Users.

Being Canadian living in Australia,

I've been so incredibly fortunate to be able to be

a part of what's been going on in Australia

over the past several years.

Australia was one of the first countries

to make hepatitis C a public health priority

and the Minister Sussan Ley made a commitment in 2015

to make access for HCV DAA therapies available

for all people living with hepatitis C.

And the government in December of that year

made hepatitis C DAA therapies available

with no restrictions based on liver disease stage,

no restrictions based on drug and alcohol use,

and enable general practitioners the ability

to prescribe new DAA therapies,

which enabled us to achieve incredibly broad access.

There's about 230,000 people living with chronic hepatitis C

in Australia, and in the first year

of DAA therapy availability

we treated almost-- over 30,000 people with day therapies

so representing about 14 percent of the estimated chronic pool.

So it's been an incredible journey.

Last year we did see a decrease

in the treatment numbers, in part

due to the fact that large numbers of individuals

who are in hospital-based settings that needed treatment

with advanced disease had been treated,

but we're placed to treat another 24,000 people with DAAs,

we estimate in 2017, which really is a tremendous effort.

And so it's really been a true success story.

Australia is now in its fifth national strategy.

so very, very strong support from the strategic perspective.

The government that actually approved

the DAA therapies and made this commitment

was actually a conservative government.

So there was actually bipartisan political support

for making the availability of DAAs to all a priority.

There was an incredibly strong role of the community;

both community hepatitis C organizations,

and also community-based drug user organizations.

And these organizations are funded by the government,

so it enables them to really have a strong voice

and they really played a key role in achieving this.

The other thing that was really, really crucial

is having the availability of guidelines

for the management of hepatitis C

so that general practitioners

could prescribe these new therapies.

There was also a very, very large education and training

component that was led by the Australasian Society for HIV,

Viral Hepatitis and Sexual Health Medicine

which was able to provide enhanced capacity

and to improve competency in both general practitioners,

drug and alcohol specialists,

and nurses and other healthcare providers working in the field.

So it's been an incredible journey

and I think it is a successful model.

One of the main differences in the response

to hepatitis C in Australia as compared to Canada

is I think Australia took very much a public health approach

to dealing with with hepatitis C.

If you take the broad access

that was available in Australia,

the negotiations that went on between government

and the role of community and providers,

and researchers into that process;

I think that was a really, really crucial component.

I think it took a little bit longer for Canada

to work together with industry, and with practitioners,

providers, the community, researchers;

to negotiate sort of better prices

on DAA therapies in Canada.

And so we're now in a situation

where some of these restrictions that are based

on liver disease stage are being removed.

Fortunately, Canada has a very strong primary care workforce

and lots of drug and alcohol providers

who are, I think, interested and willing.

And when you have restrictions based on fibrosis stage like

is in place in Canada

it actually acts as a double restriction

because if you're restricting to people

with advanced disease it's actually limiting it

to specialists who can prescribe.

So by removing this it's going to enable

the general practitioner workforce to enter in

and to assist.

I think you know Canada's incredibly well

placed to be able to get themselves on track

for the WHO elimination targets by 2030.

Currently we're not one of the nine countries

that are currently on track to achieving the WHO elimination

target, so we really need to move towards, you know...

If you look at the services that are available in Canada.

We've got a large number of community health centers,

drug and alcohol providers, engaged general practitioners.

So we just need to mobilize this workforce to be able to provide

DAA therapies and work towards

hepatitis C elimination in Canada.

Does neem treatment work for head lice

Head lice are parasites that live on the scalp hair and ears which feed on the blood it is one of the common problems

Faced by children a person or child can get head lice

Infestation through head to head contact sharing hair care products like combs and wearing clothing of people who have the head lice

Though head lice spread from one person to another they don't spread or carry any disease with them. They reproduce very quickly

A female louse can lay up to ten eggs per day

They hatch in five days and become mature to lay eggs in another five to seven days

It requires patience and regular inspection for completely eliminating these stubborn head lice

For example, they live up to six hours under the water and live up to 24 hours without human contact

One of the biggest problems about head lice removal is that lice have become resistant to many chemical based or over-the-counter solutions

Even chlorine present in swimming pools is not able to remove the head lice does need more neem oil work for head lice

Insecticidal ingredient called a Satyricon which disrupts the reproduction and growth of head lice

One neem oil with shampoo add neem oil to your regular shampoo and mix well

Apply undiluted neem oil to the hair and along the hair let it stay for a few minutes

Removal three neem oil with coconut oil the fatty content present in coconut oil

Repeat the process once in two to three days for at least one month

Neem oil with tea tree oil tea tree oil contains an insecticidal property which kills and prevents head lice

Infestation it soothes the irritation caused by scratching

olive oil suffocates the head lice

Combine one part tea tree oil and four parts neem oil add two teaspoons of olive oil and mix well

Apply the solution into the scalp and along the hair leave it on for a few minutes now

Comb the hair thoroughly to remove the lice wash your hair with shampoo and water

repeat the process once in 3 days 5 neem oil with sesame oil sesame oil contains antibacterial

And natural pesticide properties which deal with the head lice

Apart from that it also helps in hair growth

Rinse your hair with dilute apple cider vinegar and let it dry completely in a bowl mix 1/8 cup neem oil

1/4 cup of sesame oil 1 TSP tea tree oil 10 drops each of lavender oil

eucalyptus oil and rosemary essential oil

Stir the ingredients well and apply it into the scalp and along the hair

Cover the hair with shower cap and let it stay overnight in the morning comb the hair thoroughly to remove the head lice

Rinse the hair normally repeat the process once in 3 days until the hair is head lice free

6 neem oil with olive oil olive oil

Suffocates head lice by depriving them of oxygen which makes it easy to remove them by combing

Combine 2 ounces of olive oil and 15 to 20 drops of neem oil

Mix well and apply it on to the scalp and along the hair leave it on for at least 12 hours

Comb through the hair to remove the head lice rinse off normally in that dry completely

repeat the process twice in a week

Seven neem oil with tea tree oil tea tree oil contains pesticide properties, which kill the head lice

Apart from that it soothes the irritated scalp and encourages hair growth this process helps to prevent the head lice

infestation combined neem oil tea tree oil and lavender oil

pour the mixture into a spray bottle

Spray the solution onto the hair repeat the application

Daily tips and precautions if you are allergic to neem then avoid using it dry the remedies

regularly to get rid of head lice completely as head lice spread from one person to another

It is better to avoid head to head contact and share personal belongings with a person who is infested with head lice

Wash the bed sheets clothes towels hats and scarves regularly if you have an infestation

Seats in airplane theater and public transport can have head lice

So cover them with your sweatshirt or jacket before you sit clean your hair accessories and combs regularly

Repeat the process regularly which ensures the newly born head lice are removed to prevent the infestation

having a dog with epilepsy can be a really scary upsetting thing especially

when you're watching your dog having a seizure well in this video I'm going to

give you nine facts about epilepsy in dogs that I believe every owner should

know and hopefully they'll just put your mind at ease that your dog will be able

to live with this condition and will live a healthy happy life Hi I'm Dr Alex

that what we call idiopathic epilepsy or true epilepsy is a condition in dogs

that we don't actually understand why it happens in the first place

so we don't know the underlying cause but what the epilepsy results in is

abnormal electrical activity within the brain and it is this electrical activity

which results in the seizures that we see in our pets so despite not knowing

the underlying cause the second fact is that in some cases it does appear to be

genetic so certain breeds do seem to be more prone to developing epilepsy and

certain lines as well within breeds appear more prone to developing epilepsy

so this is definitely something to consider if you're looking at getting a

dog whose parents have had epilepsy or if you've got a young dog with epilepsy

and you are wondering about whether to breed from them but certainly the

suggestion would be that that might not be the best idea because their offspring

their puppies may be more likely to also have epilepsy in the future right number

three is that epilepsy in dogs develops between about six months of age and

about five years of age so that is when we typically see dogs who are going to

develop epilepsy have their first seizure if we've got a dog who's

very much younger than that or if we've got an older dog who is having seizures

then the chances are it's not truly epilepsy and they're fitting and having

seizures for another reason fact number four is that the treatment

that we give our pets to reduce their seizures is doing just that its aim is

to reduce the frequency and reduce the severity of their seizures it's not

designed to completely eliminate them entirely and there are a number of

reasons for this the main one being that

if we give really high doses that are needed to completely stop seizures then

that will increase the risk of side effects and potentially quite serious

side effects in our dogs that's not to say that the dog on treatment will have

seizures a small proportion will actually be seizure free once they're

taking that medication but just because your dog is on treatment doesn't mean

they won't be having seizures and if they are having the odd seizure then

that doesn't mean the treatment is not working now those seizures should not be

at a really high frequency and they shouldn't be too severe so if your dog

is epileptic and is having really frequent fits or if those fits are

lasting a long time they're taking a long time to recover from then

definitely a change in treatment plan may be appropriate but the actual

presence of seizures in the dog with epilepsy who's being treated isn't

necessarily anything to worry about so step number five is that the drugs that

we give to treat epilepsy in the early stages when we're starting treatment can

take some time to start working so that's something to bear in mind and

obviously your vet will discuss that with you there are different options

that we have for epileptic treatment they work in different ways and they

come at different costs and your vet will discuss with you the most likely

best option for your dog and that will be based on how bad the fits that

they've started to have are and how frequently they're having them and then

at number six is that depending on the drug that's being given very often at

the start of treatment the incidence of mild side effects is quite common so

that can be an increase in thirst drinking more or urinating more it can

also be a real increase in appetite so they just seem hungry all the time they

can become ataxic so kind of quite wobbly on their legs as though they've

had a bit too much to drink and they can also sometimes be a bit sedated in the

vast majority of cases these side effects actually wear off pretty quickly

once your dog's body becomes used to having the drug in the system if high

doses of drugs are needed to control the epilepsy then the risk of side effects

does become a little bit higher so some drugs can

affect liver function and can cause damage to the liver and it's for this reason

that we'll often recommend blood testing to check that everything's going

on as we would expect without any adverse effects as drug dose needs to

increase it may also be that a treatment plan needs to be changed so additional

drugs may need to be given or one drug stopped and another one started okay so

my seventh fact about epilepsy in dogs is that seizures are actually only an

emergency if they're lasting for more than about five minutes or if your dog

is having several seizures in a row and they're not really recovering properly

in between those seizures so those are definitely emergency situations we would

term that status epilepticus which is basically the the electrical activity in

the brain has got to such an extent that it will potentially start to cause some

damage but a short fit the vast majority lost maybe between 30 to 60 seconds

maybe up to a couple of minutes and that seems like an awful long time when

you're there watching your dog it can be very upsetting but if the seizure has

only lost in that short length of time then it's not a true emergency now if

you're wondering how you can help your dog when they're having a seizure I've

actually done a separate video about that that I'll link on here and down in

the description as well so my eight fact is the fact that stopping or missing a

dose of medication or several doses of medication can actually precipitate

seizures in your dog so if you miss a dose if you decide that your dog hasn't

had seizures in a while and you're going to try stopping the medication then that

can actually cause fits so that's definitely not something to be

recommended unless your vets discuss that with you and you're going through a

specific plan to reduce their dose to see if they can be weaned off that drug

but in general once an epileptic dog is deemed to need medication to control

their seizures and that's something that they'll need to be given for their life

and stopping by yourself or missing doses is definitely not a great idea and

then my last fact about epilepsy and dogs is the fact that when surveyed the

vast majority of owners consider that their dog still has an excellent quality

of life and this is really important so very often when epilepsy is diagnosed we

become anxious and worried about what their quality of life and what their

life span is but for owners of living with an

epileptic dog then they generally consider their dog to have an excellent

quality of life now that's not to say the owner themselves is not without

worry and there certainly is worry about how seizures will affect their dog about

medication and about side effects and that kind of thing but when it comes to

actually the well-being and the welfare of their pet then they consider that

their dog has an excellent quality of life and that's fantastic and that's

certainly something that we would expect to see so apart from epilepsy if that's

the only problem that your dog has then in between seizures they should

generally live a completely normal healthy life so those are my 9 facts

about epilepsy and dogs I hope you found them useful I hope you found them

that your dog can still be happy and healthy despite being epileptic now I've

 The UK tolerated "inexcusable" treatment of detainees by the US during the war on terror following the 9/11 attacks in 2001, a parliamentary committee has found

 The Intelligence and Security Committee found no "smoking gun" indicating that security and intelligence agencies had a policy of deliberately overlooking reports of mistreatment and no evidence that UK officers directly carried out physical mistreatment of detainees

 But it said it was "beyond doubt" that British intelligence agencies knew at an early stage that the US was mistreating detainees

 And "more could have been done" by both security agencies and Government ministers in the UK to try to influence US behaviour, the report found

 In 232 cases, UK personnel continued to supply questions or intelligence to allies after they knew or suspected mistreatment, said the ISC

 And in 198 cases, they received intelligence obtained from detainees who they knew or should have suspected had been mistreated

 There were at least 38 cases in 2002 alone of British officers witnessing or hearing about mistreatment

 The committee rejected agencies' claims that these amounts to no more than "isolated incidents", stating: "They may have been isolated incidents to the individual officer witnessing them, but they cannot be considered 'isolated' to those in Head Office

 The report also found evidence of UK officers making verbal threats in nine cases, as well as two cases in which UK personnel were party to mistreatment administered by others

 One of these has yet to be fully investigated and the committee raised the question of whether it should now be reopened

 There was no evidence that any US rendition flight transited the UK with a detainee on board, but two detainees are known to have transited through the Indian Ocean UK territory of Diego Garcia, where records of the conditions under which they were held are "woefully inadequate"

 "In our view the UK tolerated actions, and took others, that we regard as inexcusable," said committee chairman and former attorney general Dominic Grieve

 "That being said, we have found no 'smoking gun' to indicate that the agencies deliberately overlooked reports of mistreatment and rendition by the US as a matter of institutional policy

 "The evidence instead suggests a difficult balancing act: the agencies were the junior partner with limited influence, and concerned not to upset their US counterparts in case they lost access to intelligence from detainees that might be vital in preventing an attack on the UK

" ISC report  Mr Grieve acknowledged the pressure the agencies were operating under at a time when they feared an attack on the scale of 9/11 could be imminent in the UK

 "We wish to be absolutely clear that we do not seek to blame individual officers acting under immense pressure," he said

 But he added: "More could have been done at an agency and ministerial level to seek to influence US behaviour

 "More could also have been done to distance themselves from mistreatment of detainees
