One of the toughest decisions for a cancer patient is to select the right therapy to

undergo after their diagnosis.

We have take an approach to generate models for every patient and characterize their therapeutic

response using these models.

We call these models organoids because they grow just like mini organs in a dish and they

are derived from cancer cells that are taken from either surgical resection or a biopsy

taken from the patient at the time of diagnosis.

Organoids faithfully represent the genetics of the patient from which they are derived from.

This is very important because use organoids to derive signatures for therapeutic response

in patients, and we can then use these signatures to predict the therapeutic outcome of other

patients if they are provided similar drugs.

For instance, we use these organoids in the laboratory to characterize the therapeutic

response to standard-of-care chemotherapy.

This is particularly important because the options that are currently available to pancreas

cancer patients are few and they present with a lot of toxicity.

Selecting the right chemotherapy for the correct patient is extremely important because it

will give the patient the best chance at long term survival.

The hope is to take this model forward as a precision medicine tool to guide therapeutics

in the clinic.

Why the Limited Effects of Minoxidil can Wane Over Time, and More Effective Hair Loss Treatments - Duration: 8:57.

Minoxidil is a very popular hair loss treatment which is easily available, and doesn't require

a prescription.

Both men and women can use minoxidil for genetic pattern hair loss.

However, it is common for people to not find it limited in effectiveness , or feel that

it stopped working.

I'll discuss the role of minoxidil in the treatment hair loss and how I make decisions

about its use in consultation with patients about hair loss everyday in my practice.

I'm Dr Amiya Prasad.

I'm a Board Certified Cosmetic Surgeon and Fellowship Trained Oculoplastic Surgeon.

I've been in practice in Manhattan and Long Island for over 20 years.

Managing hair loss has been a significant part of my practice beginning with surgical

hair transplant and developing strategies to maximize hair coverage after transplantation.

In an effort to improve the outcomes in hair transplant surgery using a material called

extracellular matrix, a door was opened around 2011 to apply regenerative medicine technology

to nonsurgically treat male and female pattern hair loss we call TrichoStem Hair Regeneration.

This nonsurgical treatment made a significant impact on bot male and female pattern hair

loss resulting in us treating patients from around the world since 2011.

In fact, we made such an impact on female pattern hair loss which prior to our work

was treated mostly with minoxidil with limited success.

Minoxidil was originally a drug used to treat high blood pressure.

A side effect of the drug was the stimulation of hair growth which soon became more popular

for topical hair loss treatment.

Minoxidil is available over-the-counter as liquid and foam.

Custom solutions are sometimes available through doctors of but the higher percentage does

not necessarily mean more effectiveness.

A common side effect of topical minoxidil use is scalp irritation and redness.

Many men and women have found minoxidil to help with their hair coverage at least in

the short-term.

Many also find that it really makes no difference with their hair coverage.

Minoxidil does not work with everyone.

While the exact mechanism of how minoxidil works on hair is not known, it appears to

delay the normal hair shedding cycle, and extend the hair growth cycle so users have

more hair on their scalp at a given time.

It needs to be used daily to sustain the effect, and people who discontinue minoxidil often

report hair shedding soon after they stop using it.

Even with consistent daily use, hair loss continues to progress.

In my clinical experience, especially with our experience after developing TrichoStem

Hair Regeneration, I find minoxidil to be of limited value.

Minoxidil can stimulate hair growth, but most users report that growth usually consists

of fine hairs that don't grow very long, or never grow thick, and these hairs shed

easily.

In the case of male pattern hair loss, minoxidil does not affect DHT or dihydrotestosterone,

which is important in male pattern hair loss.

For women, minoxidil is one of the few hair loss treatments available.

Of the two medical hair loss therapies available for men, minoxidil and finasteride, I find

finasteride to be a more effective treatment since it blocks the formation of dihydrotestosterone,

or DHT, which in DHT-sensitive men, causes scalp hair to thin.

However, we've observed that even if finasteride can be used continuously to control DHT, it

is limited to primarily slowing down hair thinning with some regrowth.

People who come to see us typically start treating their hair loss by first buying minoxidil

and applying it.

They come in because they recognize that they are still progressing and often will report

scalp irritation.

I evaluate every patient with microscopic photos and specific standardized digital photography.

Having performed Hair Regeneration treatment since 2011, I've accumulated a lot of data

and first hand experience.

At this point I've developed a Hair Loss Profile classification system which I use

to customize treatment plans.

Factors which I look at include gender, current age, age of onset of hair loss, severity of

hair loss, medical history, and other hair loss medications.

These are used to assign the patient a hair loss profile which determines the formulations

delivery method and treatment plan.Although hair loss is progressive, with our treatment

we've been able to stimulate hair growth, prolong the hair growth cycle, and induce

thinning hair to become thicker.

As mentioned earlier, this has made a significant impact on the treatment of female pattern

hair loss which previously was treated mostly with minoxidil.

With our treatment plans in place, more than 99% of men and women see significant improvement

in their scalp coverage.

In contrast to minoxidil which requires daily application, we are able to achieve long term

results ranging up to 5 years or longer with one or two injection sessions.

Most of my patients do not stay on minoxidil after undergoing Hair Regeneration.

Some people I wean off of minoxidil to avoid shedding related to sudden cessation of use.

For male Hair Regeneration patients, I make a determination about their relative sensitivity

to DHT or dihydrotestosterone and prescribe finasteride accordingly.

Hair Regeneration, hair transplant, or any other hair loss treatment are not cures for

hair loss.

Hair loss is managed.

Hair Regeneration is about maximizing hair growth and coverage for as long as possible.

The effect of Hair Regeneration working can be seen as early as three weeks however, there

is continuous improvement which goes on for 15 to 18 months before the hair coverage appears

to be stable.

As far as time required is concerned, most patients return to work the next day.

We treat many patients who come from around the world and many actually fly home the same

day as treatment.

At a time where regenerative or stem cell-based technology such as TrichoStem Hair Regeneration

have been shown to be so effective, the role of minoxidil in the management of hair loss

is of limited value in the treatment plans I provide for my patients.

I hope you found this information helpful...thank you for your question.

Anne Wolfe's path to treatment at Vanderbilt-Ingram Cancer Center - Duration: 1:47.

- I remember the first time I even walked in the building.

I couldn't believe I was in a cancer center.

It's just hard to imagine that it's you that's walking in.

My cancer is inside the bile ducts, and not outside.

And it calls for a little bit different treatment.

I've read where some can have transplants,

but you can't with this one.

There's only one path to take.

And that's the path I took.

- This is Anne's liver when I first met her.

So my role was to try to grow that piece of liver.

We call that the future liver remnant.

So what we did was we treated the liver through the artery

into the liver with radioactive beads that kills the tumor.

When we treat the right side of the liver with the

radiation, the left side actually grows as a response.

The right side shrinks a little bit

and the left side compensates by growing.

Eventually you can in some patients get

a big enough liver remnant that they can survive on it,

and then the surgeons can operate successfully.

Sometimes we can come up with treatment options customized

to the patient and personalized to

their situation to let us help them when otherwise, there would be no hope.

(soft music)

I was recruited here in 2013

to build this program at Vanderbilt.

And our role is to contribute to the team

to present more options to patients to treat their cancer,

extend their lifespans, and have more cards in the deck

to use in order to make cancer a chronic disease

rather than a fatal disease.

- It worked well.

- [Man] Yeah, it worked well,

- It worked well, didn't it.

Athlete's Foot: Prevention and Treatment - Duration: 7:30.

Athlete's Foot: Prevention and Treatment

Athlete's foot, or tinea pedis, is a fungal infection that affects the feet in between the toes, on the sole, and sides of the feet.

The name might lead you to believe it's something that only athletes get, but anyone exposed to hot and humid conditions is susceptible.

As with any other type of fungi, it can be spread by skin to skin contact or sharing personal items.

Even exposure to contaminated surfaces like pools and public showers can transmit this fungus.

It's believed that about 98% of cases are caused by the fungi dermatophyte, and the other 2% occur when yeast such as candida or another bacteria begin to flourish.

Whatever the cause, people who suffer from athlete's foot can all agree that they need a quick solution, because the symptoms are very unpleasant and can adversely affect the health of your feet.

Are you having this problem?.

Natural treatments for athlete's foot.

Finding out whether you have athlete's foot is fairly straightforward, and the most common symptoms are usually:.

Severe itching. Strong odor.

Blisters. Moist lesions.

Pain and redness in the area.

Unfortunately, treatment isn't as easy as the diagnosis because it requires perseverance and patience to completely remove the fungus and restore your feet to good health.

Still, the earlier you start treating this problem, the sooner you can leave it behind you.

Apple cider vinegar.

This natural product contains particular acids and nutrients that regulate your skin's pH to slow the development of fungi and fight athlete's foot.

Applying it to your feet can help alleviate that annoying itching sensation, eliminate odors, and reduce redness.

How should you use it?.

Mix equal parts apple cider vinegar and water in a bowl, and soak your feet for 20 to 30 minutes while it takes effect.

Dry your feet well after this treatment, and for the best results you should repeat it every day.

Tea tree oil.

This natural product fights fungus and bacteria, and is a powerful antibiotic.

How should you use it?.

To treat athlete's foot, mix equal parts tea tree oil and olive oil, and apply it to the affected areas using a cotton ball.

Cinnamon.

Cinnamon is a spice that has antibiotic properties to improve health and fight topical bacteria and fungus.

How should you use it?.

Make a tea by boiling six cinnamon sticks in six cups of water (3 liters).

Soak your feet in the liquid for 20 minutes as it cools.

Dry your feet well, and repeat this every day.

Aloe vera.

Aloe's moisturizing and restorative properties make it an excellent option for fighting the symptoms of athlete's foot and protecting the delicate skin of this region.

How should you use it?.

Scrape the gel from a fresh leaf of aloe and apply it to the affected areas twice a day.

Complement this treatment with an antifungal agent to kill the fungus.

How can you prevent athletes foot?.

Anyone who subjects their feet to warm and wet conditions runs a higher risk of developing athlete's foot.

Getting rid of it can be difficult, so we recommend that you try to prevent it from happening in the first place.

Wear proper shoes.

Ideally you should be wearing shoes that allow your feet to breathe, preventing the buildup of moisture and heat, conditions that are prime for fungal growth.

Avoid sharing shoes and socks.

No matter how close you might be to a person, try not to share items that come in contact with your feet because it's a great way to spread fungus.

Don't walk barefoot in damp areas.

Some people have no problem walking around barefoot in places like swimming pools, changing rooms, and public showers – and this is how most of them develop this problem in the first place.

It's a good idea to always wear sandals, and to disinfect them when you get home.

Dry your feet thoroughly.

If you don't dry your feet well, the areas between your toes will remain moist, creating a perfect environment for fungus to flourish.

It's a good idea to make sure you're completely dry before putting on socks and shoes.

Choose a drying powder.

Antifungal powders are better than sprays or creams, because they help keep your feet even more dry.

Saving Money on Heart Disease Treatment - Duration: 3:54.

Saving money on heart disease treatment

what are your rights at the moment

there are no acts or laws in place to ensure that the 83,000,000 U.S. adults who are

suffering from heart disease receive help with their treatment

what problems might you run into

almost all of the tests currently use to check heart function are seen as necessary by insurance companies says Dr. Nike Goldberg

a cardiologist and medical director of the john H tisch center for women's health in New York

there aren't really any of the standard tests that you have trouble getting covered

but she did caution about some of the newer scans available

something like the latest CT scan which looks for plaque in arteries might not be guaranteed coverage and could cost $500 or $600 she sat

don't think you need to get a procedure done just because it's available and brand new

definitely talk to your Dr. and insurance company first so you aren't settled with a bill you can't pay

how can you save money on treatment

having heart disease can end up costing you a lot of money

according to the American heart association

a 2009 survey found that more than half of people suffering from heart disease had

difficulties paying for all of their treatment even if they had Health Insurance

and 46% of those who had a hard time financially delayed getting recommended care

one way you can improve your heart health is to make lifestyle changes stop smoking eat healthier exercise more

and these are things that won't necessarily cost a lot of money

it's expensive to meet one on one with a nutritionist so look into doing a group program like weight watchers Dr. Goldberg says

or see if the hospital you've got onto has nutritionists on stuff that type of service won't be covered by insurance typically

that going to a hospital nutritionist instead of one in private practice will cut down on costs

as for exercising

walking is a great way to get moving

to make sticking with your workout regimen easier

the American heart association has a program called my walking club that Peres you up with other walkers in your area

visit my walking club.org to either start a club of your own or join an existing one

a major part of treatment costs for heart disease are the prescriptions you'll be put on a combination of anticoagulants

aspirin ace inhibitors

beta blockers and more which can become very expensive

and deep liquid medicines like plaid Exar brand name and there aren't really generic options yet keeping their price high Dr. Goldberg says

that both cholesterol lowering drugs and blood pressure medications are available in generic forms so you'll likely be able to save money

there she says to make sure to shop around since these are often drugs you're going to be on for a long time if not forever

do price comparisons and see what CVS

Walgreens even Costco would charge

she sat if there's no option besides a name brand high price drug look into the partnership for prescription assistance Parks.org

the program has teamed up with most major pharmaceutical companies to offer discounted

medication to those who can prove their income is below a certain level
