Ma'am, how long have you been suffering from this ailment?

Sir, it's been 6 years.

Where else did you seek treatment for this disorder?

I took treatment from PGI, Chandigarh for 3 years.

Till the time I took the medicines I got mild relief but I never had a lot of relief.

I had to take painkillers regularly.

Since I started the Ayurvedic medicines, I felt better from the very first day.

Painkillers create a lot of complications.

Yes, painkillers are very harmful.

After how long did you get off the painkillers post starting your treatment here?

In the beginning, I had to take it once in a week then, once in 15 days.

Now, I have to take it once in a month at times.

Mr. Manish, what happens in this disorder is that our skin gets really tight.

Yes, very tight just like a stone.

It diminishes the elasticity of the skin.

Raynaud's phenomenon causes discoloration of the fingers giving it a bluish color

due to the lack of blood supply caused by vasoconstriction which constricts the blood vessels.

We correlate this with Ama (toxins) in Ayurveda which means the accumulation of toxins in the body hindering the circulation.

It involves the obstruction of channels.

We unblock those channels which promote elasticity and decrease the Raynaud's phenomenon.

Did your fingers turn blue?

Yes, sir. All my fingers would turn blue.

In winters, right?

Did you have any other problem?

I would get small wounds on my fingers, too.

And since when have you not had these symptoms?

From the time I started this treatment, these are my 1st winters.

Can you open your mouth completely?

Yes, sir.

Has the elasticity increased?

Not in my mouth but there is a lot of improvement in my hands.

Earlier, I couldn't clench my hands but now I can, to a great extent.

That means this disease caused a lot of stiffness in your body.

Yes, there's a lot of relief.

In Scleroderma, I would like to tell the patients to avoid milk and milk products.

Did you also avoid them?

Yes, sir.

This is because milk and milk products produce a lot of toxins, cause morning stiffness

Yes, mornings are very troublesome.

What is the percentage of your improvement now?

Around 50-60%

How long has it been since you started your treatment?

It's been 7 months.

It's been 7 months and she is 50% better.

Scleroderma is a complicated disorder which has a prolonged treatment.

My biggest problem was taking the pain killer which has now been stopped.

Just like she said, she took treatment from PGI for 3 years but found no effective results.

Moreover, she has also got off her painkillers.

She has also got around 60 % relief in 7 months.

And her pain has also reduced.

Yes, sir. Pain has decreased quite a lot.

For that we use Guggul for that. I gave her Kaishore guggul, Gandhak rasayana.

I would like it show it to the patients.

Kaishore Guggul, Gandhak Rasayana, Boswellia Curcumin, Rumogin 5, Hakam Churna, a few decoctions and Manjistha capsules.

I will highlight Gandhak Rasayana here as it's really good for the skin and helps retain its elasticity.

Gandhak is sulphur.

In the modern medical system, Sulphur can be given just through diet.

But, in Ayurveda we have a Sulphur based medicine called Gandhak Rasayana, it maintain and improves the luster of the skin.

Gandhak Rasayana is a classical formulation in Ayurveda.

Classical formulations are a miracle.

The person who does not get cured by anything, improves after taking the classical formulations.

For improving digestion and circulation, I have prescribed her some decoctions which are Dashmoolarishta, Punarnavarishta.

Along with this, I've given her Manjistha capsules to improve the shine of the skin.

Hakam Churna is for digestion and Rumogin5 is used to treat disorders like Rheumatoid Arthritis, it is also digestive.

All these medicines have their own importance in the treatment.

Yes. These medicines are improving her condition gradually

Pancreatic neuroendocrine neoplasms- causes, symptoms, diagnosis, treatment, pathology - Duration: 9:57.

With pancreatic neuroendocrine neoplasms, neuroendocrine refers to pancreatic cells

which release hormones in response to signals from the nerves, and neoplasm refers to a

cancer.

So, pancreatic neuroendocrine neoplasm is a cancer of neuroendocrine cells that are

within the pancreas.

They can also be called pancreatic neuroendocrine tumors, or just panNETs for short.

In addition to endocrine cells, the pancreas also has exocrine cells, which make up a majority

of the gland.

Most pancreatic cancers arise from exocrine cells and they're called adenocarcinomas,

whereas only a minority arise from panNETs.

Now, the pancreas is a long, skinny gland the length of a dollar bill which sits to

the left of the duodenum and behind the stomach, in the upper abdomen, or the epigastric region.

It plays two main roles - there's the exocrine part of the pancreas which has acinar cells

that make digestive enzymes that are secreted into the duodenum to help digest food.

There's also the endocrine part of the pancreas which has a few different types of islet cells,

or neuroendocrine cells, each of which make different hormones.

These neuroendocrine cells are present in clusters, or islands, called islets of Langerhans.

The largest group of cells are the beta (β) cells which secrete insulin.

Insulin mainly lowers the blood glucose levels by transporting glucose into the cells, and

also pushes potassium into cells, which decreases potassium in the blood.

Another group are the alpha (α) cells which secrete glucagon, it raises the blood glucose

levels by getting the liver to generate glucose from amino acids and lipids, and to break

down glycogen into glucose.

There are also delta (δ) cells which secrete somatostatin, which decreases the release

of other hormones, including insulin, glucagon, and serotonin.

There are also gamma (γ) cells, also called pancreatic polypeptide cells, which secrete

pancreatic polypeptide, which stimulates the release of digestive enzymes from the stomach

and small intestine, as well as slow down intestinal movement.

Finally, there are a few rare cell types called enterochromaffin cells, as well as D1 cells,

and G cells, which are scattered in the islets of Langerhans.

Enterochromaffin cells secrete serotonin, which helps with motility of the gastrointestinal

tract.

D1 cells release a hormone, vasoactive intestinal polypeptide or VIP, which relaxes the small

intestine and stimulates the release of intestinal digestive enzymes, but inhibits hydrochloric

acid release from the stomach.

G cells are found in the pancreas and stomach, and the secrete gastrin.

Gastrin stimulates the parietal cells in the stomach mucosa to secrete hydrochloric acid,

and also stimulates the glands in the epithelial layer.

In pancreatic neuroendocrine neoplasms, one of the many types of neuroendocrine cells

mutates and starts dividing uncontrollably, forming a tumor over time.

Sometimes the mutation is in a tumor suppressor gene like PTEN and MEN1.

Tumor suppressor genes usually regulate cell growth, so when they mutate, the result is

uncontrolled cell growth.

Mutation of the MEN1 gene results in an inherited condition known as multiple endocrine neoplasia,

or MEN, type I, where adenomas or malignant tumors form in the parathyroid gland, pituitary

gland, and pancreas.

Some PanNETs are benign tumors which means that they don't invade nearby tissues, whereas

others are malignant tumors meaning that they do invade surrounding tissues and even metastasize

or spread through the lymph to distant ones.

PanNETs are classified into functional tumors, which secrete hormones, and non-functional

tumors, which don't secrete any hormones.

Non-functional panNETs are more common, and they're usually asymptomatic until they

grow to a large size and metastasize to other organs like the liver.

Large tumors can compress surrounding structures, for example, a tumor might compress the common

bile duct, leading to a backup bile and causing obstructive jaundice, which is when the skin

turns yellow from the buildup of bilirubin in the tissues.

Functional panNETs are less common and depend on the type of cell they arise from and the

hormone they produce.

The most common functional panNET is an insulinoma which arises from β cells that make insulin.

Insulinomas are usually benign solitary tumors that cause hypoglycemia, or low blood glucose,

and in severe cases can lead to confusion and loss of consciousness.

The second most common type is a gastrinoma that arises from the G cells that make gastrin.

Gastrinomas secrete large amounts of gastrin, which results in excess hydrochloric acid

secretion, which produces a syndrome called Zollinger-Ellison syndrome.

Excess hydrochloric acid erodes the mucosa, which results in peptic ulcers.

A unique feature of Zollinger-Ellison syndrome is that the peptic ulcers are formed not only

in the stomach, and duodenum, but there's so much acid that ulcers are also formed in

more distant parts of the small intestine like the jejunum.

The acid also inactivates the pancreatic digestive enzymes, which allows food, especially fat

to pass right through the intestines undigested, leading to greasy and smelly stools—called

steatorrhea.

The third most common type of panNETs is a VIPoma which arises from the D1 cells that

make vasoactive intestinal peptide.

VIPomas result in WDHA syndrome, where WD stands for watery diarrhea, which is due to

increased fluid secreted by the small and large intestine, H stands for hypokalemia,

or low potassium levels in the blood due to potassium losses in the intestinal fluid,

and A stands for achlorhydria, the absence of hydrochloric acid secretion.

The fourth type is the glucagonoma which arises from the α cells that make glucagon.

Excess glucagon leads to increased blood glucose, resulting in diabetes mellitus.

Glucagonomas also result in a loss of lipids and proteins that are used to form glucose,

and they typically cause a necrolytic migratory erythema, which is a red blistering rash that

mostly affects the mouth and limbs.

The fifth type is a somatostatinoma that arises from δ cells which secrete somatostatin.

The excess somatostatin inhibits release of other pancreatic hormones, like insulin, gastrin

and VIP; which leads to diabetes mellitus, steatorrhea, and hypochlorhydria, or decreased

hydrochloric acid secretion.

Finally, there are also panNETs that develop from pancreatic polypeptide cells and enterochromaffin

cells, but these are very rare.

Diagnosis of functional panNETs is based on measuring pancreatic hormone levels.

Non-functional tumors are usually hard to detect since no increase hormone levels can

be found in the blood.

In addition, MRI or CT imaging of the pancreas can help identify a tumor and estimate its

size.

The treatment for a panNETs depends on the type and size.

When dealing with a functional tumor, with the exception of somatostatinomas, somatostatin-like

medications are given to reduce hormone production.

In addition, gastrinomas can be treated with medications that block gastric acid secretion.

When dealing with nonfunctional panNETs, large tumor that cause compression can be managed

with surgery.

All right, as a quick recap, pancreatic neuroendocrine neoplasms, or panNETs, are tumors of hormone

producing cells of the pancreas, and their symptoms depend on their size and cellular

origin.

PanNETs can be non-functional tumors, meaning they don't produce any hormones, or they can

be functional tumors, meaning they produce hormones.

The most common types of functional tumors include insulinomas that make insulin, gastrinomas

that make gastrin, glucagonomas that make glucagon, VIPomas that make vasoactive intestinal

peptide, and somatostatinomas that make somatostatin.

- Strabismus is any misalignment of the eye.

It can be either eye crossing,

which is called esotropia,

or eyes that are drifting,

so-called exotropia.

There are different ways for us to fix this.

We can try glasses.

Whenever the kids see better,

their brain is gonna pull the eyes together

and use them together.

But if they don't see better,

then we can do eye muscle surgery

where we adjust the locations of the muscles

to help them keep their eyes straighter.

What we want to do is we want to get the vision better.

Treatment of Primary Liver Cancer - Intrahepatic Cholangiocellular Carcinoma - Duration: 4:29.

Primary liver cancer is

a highly malignant carcinoma.

It can hardly be detected at its early stage,

and while in its medium or advanced stage,

to remove it using surgery become more difficult,

and victims are very likely to

experience its recurrence and metastasis.

Two years ago, Ms Huang was diagnosed to be with primary liver cancer,

and she was in great danger.

How did the doctors fight against this cunning disease

and help Ms Huang with her narrow escape from the shadow of death.

55-year-old Ms Huang

is an office worker from Guigang City, Gungxi Zhuang Autonomous Region.

Back in June, 2013,

abrupt acute abdominal pain

disrupted her peaceful life.

It was so painful that I had keep my knees against the chest.

It occurred every day, even in every night.

I had to receive painkiller injection.

But before long,

even painkiller injection became ineffective.

Only with such an odd gesture

could she barely fall asleep.

And a few minutes later,

she was again awaken by the tortuous pain.

Hou Encun, chief physician of

Ruikang Hospital affiliated to Guangxi University of Chinese Medicine,

was one of the doctors in charge of Ms Huang`s treatment.

Shortly after a series of examination,

he discovered the cause for the pain.

In the examination,

we noted that her upper abdomen

was tender, quite apparently so.

In the CT scan that followed,

we discovered a considerably large lump

between her left liver lobe and right liver lobe.

When we conducted puncture biopsy

and the pathological report revealed

it`s a kind of primary liver cancer,

namely intrahepatic cholangiocellular carcinoma.

Primary liver cancer

is a highly malignant carcinoma.

It grows rapidly

and metastasis is easily developed.

It was this tumor

that caused acute pain afflicting Ms Huang.

This is the CT scan

film taken when Huang Weihong

was admitted here for treatment.

It shows her liver tissue.

And then another tissue,

which is actually the tumor.

Her viscera were apparently under compression,

and that is because it grew rapidly

and was causing considerable damage

to the tissues around it.

This explains the severe pain.

How to curb the growth of the tumor

and relieve Ms Huang of pain?

Hou Encun quickly devised a scheme for treatment.

We proposed to use cyber knife

which is minimally invasive.

High-precision radiotherapy using X-ray

inhibited replication of cancer cells.

Apoptosis occurred with the size of cancer cells becoming smaller,

as an option for cancer treatment.

Radiation is one of the ways widely used

today for killing cancer cells,

and is more commonly known as radiotherapy.

Thanks to cyber knife with high precision,

the tumor inside Ms Huang`s liver was soon curbed.

The treatment lasted 4 days.

The time when it was finished,

the patient felt her pain was much alleviated.

And 3 to 4 day later,

the pain was virtually gone.

After the cyber knife therapy,

I felt so relieved that

it was as if a stone on my chest had been removed.

My entire body was relaxed.

That was how I felt.

The fact that the pain was alleviated relieved Ms Huang.

However, to the doctors,

It did not necessarily mean

successful completion of the treatment.

It was a local treatment after all.

In surgery, only part of the lesion is cut and removed.

And in cyber knife therapy, we make no exception.

There can be potential metastasis lesions

or tumors that are yet to develop.

That is why we need integrate it

with other options for a comprehensive treatment.
