ACL Rupture: Nonoperative Treatment - Hamilton Bone & Joint Clinic - Duration: 1:31.

ACL stands for anterior cruciate ligament. This is an important structure

within the center of the knee that connects the femur to the tibia and

keeps the tibia from sliding forward on the knee.

When a person ruptures their ACL this is usually a very high-energy injury, the

patient usually hear a pop - people around them may actually hear a pop - and it can be

very difficult to determine whether or not this is an isolated ACL injury or

actually involves a fracture. In the acute setting, a patient who has this

mechanism of injury - this intensity of injury - needs to seek emergency medical

attention to exclude a more dangerous fracture.

However, if fractures have been excluded and the ACL is the only isolated injury

patients can generally progress to weight-bearing on this. The important

early non-operative management of an ACL injury involves regaining the strength

and the range of motion of the knee. This can be done with physical therapy. Some

people do very well without an ACL; some people do very poorly without an ACL. The

ACL again provides stability to the joint. In patients where the stability of

their knee, without the ACL, is allowing them to do the activities that they

desire to do, nonoperative treatment can be pursued.

Shoulder Dislocations: Nonoperative Treatment - Hamilton Bone & Joint Clinic - Duration: 1:31.

Nonoperative treatment of a shoulder dislocation is the primary goal in a lot

of situations. However in the acute setting if someone suspects that they

have a shoulder dislocation this is an emergency and they need to seek medical

attention immediately. Once the shoulder is relocated, some of

these can be managed successfully without surgery. This depends upon the

patient's goals. Factors that are associated with increased risk of redislocation

include male gender, younger age patients, and patients that are engaged

in contact sporting activities. Patients that are good candidates for

nonoperative treatment of shoulder dislocations are older patients, patients

who are first time dislocators of the shoulder and patients who can avoid

high-risk activities. When a shoulder dislocation occurs there's necessarily a

tear of the capsule and the ligament that support the shoulder. This pulls

away from the socket and will not heal in the correct position unless this is

fixed. The decision as to whether or not to fix this depends upon the patient's

goals. The biggest risk with nonoperative treatment is the

recurrence of the dislocation. One out of three patients will experience a redislocation

after a first-time dislocation. The danger with this is additional

injury to the structure surrounding the shoulder.
